Immunization Reminder for 7th-Grade Families

The following message was sent to incoming 7th-grade families: 

Dear Parents/Guardians,

Tennessee state law requires that your student has a booster dose of tetanus-diphtheria-pertussis vaccine (known as TDAP) to be admitted into the 7th grade next school year. This requirement must be documented on the official Tennessee Certificate of Immunization form. Out-of-state records are not acceptable except for active Military or National Guard.

Please make arrangements with your healthcare provider, the Health Department, or an immunization partner for your child to receive this new immunization before the first day of the 2024-2025 school year.

Per state law, no child can be permitted to attend any public school until proof of immunization is presented. If your student qualifies for a Medical or Religious Exemption, documentation must be provided. Please click here to learn more.
